Fortify Wyoming LLC is a Wyoming case management agency serving participants of the Wyoming Medicaid Community Choices Waiver. We support eligible older adults and individuals with disabilities by helping coordinate services that promote safety, independence, and quality of life at home.
Our approach is person-centered, practical, and community-focused. We understand that each participant has unique goals, health needs, family dynamics, risks, routines, and preferences. Our job is to help bring those pieces together into a clear service plan that supports the participant’s daily life.
