Support for individuals and families across Wyoming
The Community Choices Waiver assists older Wyoming adults age 65 and older who are on or eligible for Medicaid. It also supports individuals ages 19 to 64 with a confirmed disability that meets Social Security Administration criteria.
Older Adults
We help older adults who want to remain at home or in a community setting while receiving the services and supports they need to stay safe.
Adults With Disabilities
We support adults with disabilities who need help coordinating care, managing services, and accessing resources that promote independence.
Families and Caregivers
We help families understand the waiver process, communicate with providers, and support their loved one’s care plan.
Participants Transitioning From Institutional Care
Some participants may need support transitioning from an institutional setting back into the community. The Community Choices Waiver includes transition-related services when approved and appropriate.
