Wyoming Community Choices Waiver Case Management

The Wyoming Medicaid Community Choices Waiver helps eligible older adults and adults with disabilities receive home and community-based services instead of moving into an institution.

The waiver supports individuals who meet program requirements related to Medicaid eligibility, level of care, and financial criteria. Fortify Wyoming LLC provides case management services that help participants move through the process, coordinate care, and maintain the supports they need to live safely in their home or community.

What We Help With

Comprehensive Assessments

We help review participant needs, strengths, goals, risks, home safety concerns, health-related needs, and available supports.

Person-Centered Service Planning

We help create a service plan that reflects the participant’s preferences and priorities while addressing health, safety, and daily living needs.

Coordination of Waiver Services

We help connect participants to available CCW services such as homemaker services, personal support, respite, transportation, home-delivered meals, assisted living facility services, skilled nursing, and other approved supports when appropriate.

Provider Communication

We help coordinate with service providers, healthcare professionals, family members, and community resources to keep the care plan moving forward.

Routine Monitoring

We conduct ongoing reviews and home visits to make sure services are being delivered, needs are being met, and the participant remains safe.
