Fortify Wyoming LLC provides case management services for eligible participants of the Wyoming Medicaid Community Choices Waiver. Our services are designed to help participants access the right supports, understand their care plan, and remain safe in their home or community.
We help evaluate the participant’s current needs, goals, risks, abilities, and support system. This may include reviewing health and safety concerns, daily living needs, home environment, caregiver support, and community resources.
Every participant deserves a plan that reflects their voice. We help create person-centered plans that support self-determination, independence, and safety. The plan may include waiver services, natural supports, provider coordination, and strategies to reduce identified risks.
We help connect participants to appropriate services and supports. Depending on the participant’s assessed needs and approved plan, this may include coordination with personal support providers, homemaker services, respite care, transportation, medical services, community programs, and other local resources.
Needs can change over time. We provide ongoing monitoring through routine contact, home visits, service reviews, and communication with providers and families. Our goal is to help ensure the service plan remains effective and responsive.
Families and caregivers often play an important role in a participant’s care. We help explain the process, clarify next steps, and support communication between the participant, providers, and support network.
